The upper part of each leaf is shaped like a church dome. The icon is enameled in light blue. The leaves are joined by convex round hinges. At the center is an image of the Mother of God. The Mother of God, "Joy of All Who Sorrow," is shown full-length with arms outstretched. She is surrounded on both sides by the sick, the crippled, and those afflicted at heart. To the left and right of the Mother of God's halo are inscriptions from a prayer to the Theotokos. On the leaves are scenes of the Twelve Great Feasts: on the left, "The Entry into Jerusalem" and "The Presentation of the Lord"; on the right, "The Descent into Hell" and "The Ascension"; in the kokoshnik, "God the Father, Lord of Sabaoth."
